He he’s been to the ER for the usual symptoms (lower back and flank pain, blood in urine, fever, chills) and the urologist we took him to a few days ago scoffs at him.

2mm is a small stone. The amount of pain you can experience doesn’t seem to much effected by the size of the stone, however at that size hopefully it’s small enough so pass on its own quickly. The only real advice is drink a lot, take painkillers, and consult your doctor if you have any concerns.
